Medical Records FAQs

Q: How do I upload a request?

Visit https://chartstream.urpt.com/request-app/ and click on “new request.” Complete all required fields.

Q: How do I know if my request has been received?

You will receive a confirmation email with a request ID.

Q: How do I check the status of my request?

Please click the tracking link included in the confirmation email you received after submitting a request and enter the tracking ID. https://chartstream.urpt.com/request-app/

Q: What do I do if my request was rejected?

If your request was rejected, you will receive an automatic email stating the reason for the rejection. The rejection reason will also be visible from the tracking page. If additional information is needed, please submit a new request with the required information. For additional questions, please contact the ROI department.

Q: How will I receive my records?

Once the records have been processed, you will receive an email with a downloadable link(s) to both the requested records and a separate link for the invoice. Please note all record links expire within 30 days.

Q: How do I make payment?

All invoice information is listed on the invoice received with the records. Please either call 844-319-6137 to make a card payment or mail a check to the address listed at the top left of the invoice.

Q: The link to my records is expired. What do I do?

All dispatched records contain a 30-day expiration as an extra security measure to ensure the privacy of all patient information. If 30 days have passed since the records were emailed, please submit a new request to https://chartstream.urpt.com/request-app/

Q: How do I raise an issue with my request?
